There are three questions that players, staff, and spectators have to answer before they can attend an event.

1) Have you or anyone you've been in close contact with had a sore throat, cough, chills, body aches, shortness of breath, loss of smell, loss of taste, fever at or greater than 100° Fahrenheit (38° Celcius) in the last 14 days?

2) Have you or anyone in your household tested positive for COVID-19 in the last 14 days?

3) To the best of your knowledge have you been in close proximity, in the last 14 days, to any individual who tested positive for COVID-19?
